Job summary

Angels Care Home Health in Denison, IA is seeking an experienced LPN Intake Coordinator to join our interdisciplinary team. You will manage referral intake and coordinate patient care using the Home Care Home Base system, ensuring smooth handoffs and timely processing.

The successful candidate will have an LPN license, a valid driver's license, and at least 1 year of home health experience; hepatitis profile proof is required.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Driver's license for the state.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• Coordinate intake referral process with interdisciplinary team.
• Process referral intake via Home Care Home Base software system.
• Ensure all referral and intake documents are complete and accurate.
