MUSC Community Physicians (MCP) seeks an LPN to provide patient-centered nursing care under the supervision of a registered nurse. You will assist with data collection, care plans, and evaluating outcomes within a defined patient population.

Responsibilities include interviewing patients, documenting in the medical record, performing screenings, and supporting the care team with timely communication and accurate data entry.

Qualifikationen

• Graduation from a recognized Licensed Practical Nursing program and licensed as an LPN within SC or a compact state.
• One year of LPN experience preferred.
• Current Basic Life Support (BLS) required, either AHA BLS for Healthcare Providers or American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.

• Provides patient/family centered care, acting as partner and adviser, assisting patients and families to manage their health care respecting culture and values.
• Uses clinical judgment to expedite appropriate care, including for complex or potentially life‑threatening conditions.
• Documents findings and plans in the medical record per policy.
• Performs telephone and in‑person screenings and records vital signs.
• Prioritizes and documents patient calls regarding medications and health conditions.
