Swanton Valley Rehabilitation and Healthcare Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our nursing team on Night Shift.

As an LPN/RN at Swanton Valley, you will provide quality nursing care to our residents while helping maintain a safe, comfortable, and resident-centered environment. Our nurses play an important role in leading the care team and ensuring residents receive the attention and clinical support they deserve.
Provide direct nursing care in accordance with residents' plans of care
Administer medications and treatments as ordered
Complete nursing assessments and monitor changes in resident condition
Communicate changes and concerns promptly to physicians and appropriate members of the care team
Accurately complete required clinical documentation
Supervise and support STNAs during the shift
Assist with admissions, transfers, and discharges as needed
Respond appropriately to changes in condition and emergency situations
Maintain compliance with facility policies and applicable state and federal regulations
Promote a positive, respectful environment for residents, families, and coworkers
Current LPN or RN license in the State of Ohio in good standing
Long-term care or skilled nursing experience is preferred, but not required
Strong clinical judgment, communication, and organizational skills
Ability to work effectively as part of an interdisciplinary team
Dependable, compassionate, and committed to providing excellent resident care
