Job summary

Qualifications

Responsibilities

• Provide direct nursing care according to residents' plans of care.
• Administer medications and treatments as ordered.
• Complete nursing assessments and monitor changes in residents' conditions.
• Communicate changes to physicians and appropriate care team members.
• Accurately complete required clinical documentation.
• Supervise and support STNAs during the shift.
• Assist with admissions, transfers, and discharges as needed.
• Respond to changes in condition and emergency situations.
• Maintain compliance with facility policies and regulations.
• Promote a positive, respectful environment for residents, families, and coworkers.

Swanton Valley Rehabilitation and Healthcare Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our nursing team on Night Shift.

Position Overview

As an LPN/RN at Swanton Valley, you will provide quality nursing care to our residents while helping maintain a safe, comfortable, and resident-centered environment. Our nurses play an important role in leading the care team and ensuring residents receive the attention and clinical support they deserve.

Responsibilities

• Provide direct nursing care in accordance with residents' plans of care
• Administer medications and treatments as ordered
• Complete nursing assessments and monitor changes in resident condition
• Communicate changes and concerns promptly to physicians and appropriate members of the care team
• Accurately complete required clinical documentation
• Supervise and support STNAs during the shift
• Assist with admissions, transfers, and discharges as needed
• Respond appropriately to changes in condition and emergency situations
• Maintain compliance with facility policies and applicable state and federal regulations
• Promote a positive, respectful environment for residents, families, and coworkers

Qualifications

• Current LPN or RN license in the State of Ohio in good standing
• Long-term care or skilled nursing experience is preferred, but not required
• Strong clinical judgment, communication, and organizational skills
• Ability to work effectively as part of an interdisciplinary team
• Dependable, compassionate, and committed to providing excellent resident care
