Aspire Senior Living Warsaw in Missouri is seeking a dedicated LPN/RN MDS Coordinator to lead the care planning process for residents. You will collaborate with the interdisciplinary team to develop and implement individualized care plans, perform assessments, and maintain accurate records.

Responsibilities include initiating care plans with the team, scheduling planning meetings with residents and families, tracking progress, and contributing to quality improvement efforts to enhance resident

Qualifications

• Current LPN/RN license in Missouri
• Experience in long-term care or related field
• Strong assessment and care planning skills
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced environment

Responsibilities

• Conduct initial and ongoing assessments of residents' needs and abilities
• Develop and implement individualized care plans in collaboration with the care team
• Coordinate and facilitate care planning meetings with residents, family members, and healthcare professionals
• Monitor and document resident progress, ensuring that care plans are updated and revised as needed
• Collaborate with the care team to identify and address resident concerns and issues
• Maintain accurate and complete records of resident assessments, care plans, and progress
• Participate in quality improvement initiatives to enhance resident outcomes and care
