Job summary

Behavioral Health Network (BHN) seeks a Licensed Practical Nurse (LPN) to join its Respite team serving Holyoke and Westfield. The LPN will deliver direct nursing care, perform intake assessments, administer medications, and support aftercare planning in a community-based setting.

The role requires MA LPN licensure, at least 1 year of behavioral health experience, and strong communication with providers. Competitive benefits and a mission-driven culture are offered.

Qualifications

• LPN licensure in the Commonwealth of Massachusetts.
• Minimum 1 year of training in behavioral health and 1 year of post-graduate nursing experience.
• High school diploma or GED.

Responsibilities

• Provide direct clinical and nursing services to individuals in the Respite program.
• Administer, transcribe, and verify medications; document progress and treatment plans.
• Collaborate with Nurse Manager and providers on referral/treatment planning and medical interventions.
• Perform comprehensive nursing assessments and document findings.
• Complete paperwork including nursing assessments, progress notes, medication orders, and discharge summaries.
• Provide MAP-competency training to staff and maintain program documentation.
