Job summary

Qualifications

• High School Diploma or GED.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational / Practical Nurse.
• 1 year of experience in a clinical care setting.
• Experience in hospice or home health preferred.
• 50% travel required.

Responsibilities

• Verifies Plan of Care prior to each visit and provides care per RN guidance.
• Record medication and update patient profiles per procedures.
• Communicate significant changes to RN Case Manager and physician.
• Support patient safety using family and community resources.
• Assist patient learning self-care techniques during visits.
• Collaborate with RN Case Manager during business hours for advice.
