Job summary

South Creek is hiring a Licensed Practical Nurse (LPN) in Centralia, WA. The role involves hands-on nursing care for residents in post-acute rehab and long-term care, medication administration, and collaborative care with RNs and CNAs. Strong documentation in the EHR and good communication are essential.

Qualifications

Current Washington LPN license.
CPR/BLS certification.
Strong communication and teamwork.
Experience in skilled nursing or long-term care is a plus; new grads welcome.

Responsibilities

Provide hands-on nursing care to residents in post-acute rehab and long-term care.
Receive and administer medications and treatments safely and accurately.
Monitor changes in residents' conditions and inform RN/care team.
Maintain accurate documentation in the EHR and support CNAs on the unit.
