Job summary

Oakview Health & Rehab is a non-profit center in Georgia seeking a licensed practical nurse (LPN) to support assessment and care planning. The role focuses on coordinating care plans, communicating with patients and families, and collaborating with interdisciplinary teams to ensure high-quality nursing care.

You will assist with admissions, review patient data for needs, and help educate the team on care plans, while complying with federal and state regulations.

Qualifications

• Nursing degree or LPN program.
• Active LPN license to practice in Georgia.
• Proficient with Microsoft Office applications (Word, Excel, PowerPoint).
• Strong organizational and communication skills.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to support assessment/care planning.
• Coordinates interviews and collects information from patients, families and staff.
• Assists Director with patient condition evaluation and admission assessments.
• Monitors care plan development and ensures timely completion and signatures.
• Ensures compliance with regulations and educates staff on care plans.
