Job summary

Oakview Health & Rehab in Georgia seeks a licensed practical nurse (LPN) to deliver compassionate, patient-centered care within a mission-driven nonprofit setting.

You will collaborate with the interdisciplinary team to assess patients, develop and monitor care plans, and ensure regulatory compliance while providing high-quality care during evening, night, and weekend shifts with differential pay.

Qualifications

• Nursing degree or graduation from an approved LPN program.
• Active LPN license to practice in Georgia.
• Proficient with Microsoft Office and basic computer skills.
• Strong organizational, communication and interpersonal skills.
• Ability to read and interpret documents and write routine reports.

Responsibilities

• Facilitates communication between nursing center departments and patients/families to develop care plans.
• Assists Director with patient interviews, assessments and evaluation of conditions.
• Coordinates development and execution of preliminary/comprehensive care plans.
• Ensures care plans are completed timely and signed with dates and accuracy.
• Supports MDS process education and interdisciplinary teamwork for patient care.
