Job summary

Harbor Healthcare System in Independence, MO is seeking a full-time Quality Assurance LPN/LVN to join our team. The role focuses on quality control of health services and reviewing documentation for compliance and appropriateness of care.

The ideal candidate will have an AA degree, active LPN/LVN license, at least one year of hospice or home health experience, strong communication, and the ability to travel as needed.

Qualifications

• Must have an AA degree or equivalent from a two-year college.
• Must hold a current State LPN or LVN license.
• Experience in hospice or home health preferred.
• Strong written and verbal communication required.
• Valid driver's license and ability to travel as needed.

Responsibilities

• Ensure quality control of all health services provided by the Agency.
• Review documentation for compliance and appropriateness of care.
• Review nurses’ notes for completeness and adherence to orders.
• Review admission packets and POC/recertification for accuracy and updates.
• Conduct in-services for documentation problems as needed.

We are currently seeking qualified candidates for a full time Quality Assurance (QA) LPNNurse positionin the Independence area.

Job Description:

A Quality Assurance LPN/LVN is responsible for assuring quality control of all health services provided by the Agency and for reviewing documentation for compliance and appropriateness of care.

Qualifications:

• Must have an associate degree (A.A.), or equivalent from a two (2) year college.
• Must have at least one year’s experience in hospice, home health or a related field.
• Must have a current State LPN or LVN license.
• Must be able to speak and write effectively.
• Must work well with other disciplines to coordinate patient care.
• Must be able to prepare forms for pre and post payment audits.
• Must have a current State driver's license
• Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, regulations required
• Experience in reviewing admission packets required
• Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan
• Conducts one-on-one in-services for documentation problems as needed
• Effective written and verbal communication
• Ability to travel - must havereliable transportation
• Clean background and drug screen
