Job summary

Harbor Healthcare System seeks a full-time Quality Assurance LPN/LVN in the Houston area to assure quality control of health services and review documentation for compliance and appropriateness of care.

The role requires an associate degree or equivalent, an active LPN/LVN license, and at least one year in hospice or home health. Strong communication, audit prep, and travel readiness are essential.

Qualifications

• Must hold an Associate degree or equivalent from a two-year college.
• Must have a current State LPN or LVN license.
• Ability to speak and write effectively.
• Experience in hospice or home health settings preferred.
• Strong documentation review and audit preparation skills.
• Must have reliable transportation for travel.

Responsibilities

• Review documentation for compliance and appropriateness of care.
• Review nurses’ notes for completeness and adherence to orders and regulations.
• Review admission packets and POC/re-certification for accuracy and updates.
• Conduct one-on-one in-services for documentation problems as needed.
• Coordinate patient care with other disciplines.
• Prepare forms for pre and post payment audits.
• Travel as required with reliable transportation.
• Maintain clean background and pass drug screens.
