Who we are looking for

Licensed Practical Nurses (LPNs) or Physical Therapy Assistants (PTAs) — including new graduates — who want to make an impact in skin and wound care through in‑home patient visits. You bring the passion—we’ll provide the training and support!

Main Responsibilities (may include but are not limited to)

• Traveling independently to private homes, adult family homes, and independent living facilities
• Independently perform wound care, including photo documentation of skin conditions, dressing changes, and UltraMist treatments (pain‑free debridement) – seeing approximately 6‑8 patients a day
• Apply and remove topical medications and dressings
• Collaborate with our providers virtually through audio/visual visits
• Occasionally assist the provider in person with wound care procedures
• Perform administrative tasks and complete documentation as necessary
• Abide by company safety protocols for every visit; i.e. check in and check out
• Participate in regular team meetings for training, team building, and professional development
• Perform additional tasks and responsibilities as assigned

Skills required to succeed

• LPN or PTA; licensed in working state
• Driving! Must have reliable transportation, a valid driver’s license, and current auto insurance
• Ability to travel to multiple locations each day within an assigned territory
• Telehealth experience – preferred
• Ability to multitask while providing compassionate patient care
• This fast‑paced position requires standing for extended periods of time
• Computer savvy; intermediate proficiency using MS Suite: Word, Teams, Outlook, and Excel
• Leadership qualities with the ability to work well within a team of healthcare providers and partner facilities
• Must be self‑motivated, punctual, dependable, and able to work/schedule appointments independently
• Excellent attention to detail, strong organizational skills, and ability to manage time effectively
• Ability to respond appropriately to constant change in priorities and services
• Must have strong interpersonal skills and the ability to communicate effectively with a variety of people through both verbal and written communication lines
• Must be trustworthy, honest, and have a positive and professional attitude

Compensation

$43.00 – $50.00 – DOE and territory with bonus potential

Hourly, non‑exempt; full‑time employment

Hours

Typical hours are Monday through Friday; varied hours between 8 a.m. – 5 p.m. PST, no nights or weekends.
