Job summary

Vitality Living Elizabethtown is seeking an LPN/Charge Nurse to join our community in Elizabethtown, KY. You will assist with medication administration, coordinate care plans, and communicate with physicians to ensure accurate charting and timely treatment.

Responsibilities include maintaining licenses, CPR, and state-required credentials while supporting residents and families with compassionate, professional care within a vibrant team. This is an on-site role at our Elizabethtown community.

Qualifications

• Must be at least 21 years old.
• Communicate effectively in English, both verbally and in writing.
• Meet state health related requirements.
• Maintain current professional license and CPR certification if currently LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

• Collect, store, and ensure transport of certain specimens for laboratory studies
• Communicate with physicians via telephone, fax, and Physician Communication forms
• Ensure physician orders, laboratory reports, and other data is scanned and filed in resident charts
• Assist in the development and revision of service plans. Inform Wellness Director of current resident needs and preferences as well as successful service approaches
• Perform other duties as directed by Wellness Director
