Job summary

MFM Health in Danvers, MA is seeking a compassionate full‑time Licensed Practical Nurse (LPN) to join our primary care team as Provider Support & Clinical Coordinator. You will coordinate clinical workflows, assist with documentation, and support providers to improve patient care and efficiency.

Qualifications

• Must hold an active Licensed Practical Nurse (LPN) license in Massachusetts.
• Experience in primary care or ambulatory care is preferred.
• Experience with electronic medical records; Athenahealth preferred.
• Strong organizational, multitasking, time management, and communication skills.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Support efficient daily clinical operations alongside assigned physicians and providers.
• Act as a clinical scribe documenting encounters in the EMR.
• Review patient charts and prepare for appointments to ensure completeness.
• Scrub provider schedules to identify care gaps and follow‑up needs.
• Assist with medication management including refills and prior authorizations.
• Coordinate referrals, diagnostic tests, and outside medical records.
• Facilitate communication between patients, providers, and ancillary services.
• Provide coverage and support for Medical Assistants and nursing staff.
