Job summary

MFM Health in Danvers, MA, is seeking a full-time Licensed Practical Nurse (LPN) to support providers and coordinate clinical workflows. You will document encounters in the EMR, manage patient charts, and help optimize daily operations in a fast-paced primary care setting.

Qualifications

• Must hold current Licensed Practical Nurse (LPN) license in the Commonwealth of Massachusetts.
• Experience in primary care, family medicine, or ambulatory care preferred.
• Experience with electronic medical records; Athenahealth experience preferred.
• Strong understanding of clinical workflows, care coordination, and provider support.
• Excellent organizational, multitasking, and time-management skills.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with providers and multidisciplinary teams.
• Demonstrated critical thinking and adaptability in a fast-paced clinical environment.

Responsibilities

• Coordinate clinical workflows with physicians and advanced practice providers to support daily operations.
• Scribe during patient visits, documenting encounters in the EMR.
• Review patient charts prior to appointments and identify outstanding clinical needs.
• Scrub provider schedules to identify care gaps and overdue preventive services.
• Assist with provider inbox management and timely completion of clinical tasks.
• Support medication management including refills, prior authorizations, and questions.
• Review labs and imaging reports for appropriate routing and action.
• Provide coverage for Medical Assistants and nursing staff as needed.
• Collaborate with care teams to ensure continuity of care and patient navigation.
