Valor Healthcare, Inc. is seeking a dedicated Licensed Practical Nurse to join our Community Based Outpatient Clinic in East Los Angeles. The role offers a competitive salary, excellent benefits, generous time off, and a weekday schedule.

You will collaborate with the PACT Team and VA partners to develop patient care plans, provide nursing care, and support enrollment in services. A current LPN/LVN license, BLS, ACLS and EMR experience are required for the position.

Qualifikationen

• Diploma or certificate of an accredited practical/vocational nursing program required.
• Minimum one-year experience as LPN/LVN in primary care or ambulatory setting; government healthcare environment preferred.
• Must be licensed/credentialed and in good standing with VA requirements.
• Current BLS certification and ACLS as required by VA contract.
• Proficiency in English and EMR use.
• Strong computer skills and ability to work with clinical documentation.

Aufgaben

• Provide prescribed nursing care and personal care to patients in the clinic.
• Perform nursing assessments, vitals, and collect patient information.
• Administer medications as ordered and document in electronic medical records.
• Operate diagnostic equipment (EKG, glucometers, etc.) and assist with tests.
• Maintain infection control and clean treatment rooms and equipment.
• Deliver patient education focusing on self-management and wellness.
