Valor Healthcare, Inc. in New Braunfels, TX is seeking a Licensed Practical Nurse to join the Community Based Outpatient Clinic team. You will provide prescribed medical treatment and personal care under physician guidance, and collaborate with the PACT Team to support patient care plans.

Formación
- Diploma or certificate of a practical nursing program approved by the state.
- Minimum one-year experience as an LPN/LVN in primary care or ambulatory care.
- Must be credentialed and in good standing with the VA (as applicable).
- Current BLS and ACLS certifications.
- Proficiency in written and spoken English.
- Strong computer skills and EMR experience.
Responsabilidades
- Assist with patient care, exams and ancillary tests.
- Perform nursing assessments and preventive services; document in EMR.
- Administer medications as ordered and document times and dosages.
- Operate diagnostic equipment and collect lab specimens.
- Maintain cleanliness of treatment areas and ensure confidentiality.
- Provide patient and family health education and coordinate with care team.
