Job summary

Valor Healthcare, Inc. in Denton, TX is seeking a Licensed Practical Nurse to join the Community Based Outpatient Clinic. You will provide prescribed medical treatment and personal care to veterans, working within the PACT team and coordinating with VA and community partners.

Candidates must hold an LPN/LVN license, have at least one year of related experience, and maintain BLS (ACLS as required). The position offers a competitive salary, excellent benefits, generous time off, and a weekday

Qualifications

• Current LPN/LVN license in the state of employment
• Minimum 1 year of LPN/LVN experience in primary care or ambulatory settings (VA experience preferred)
• BLS certification; ACLS as required by contract
• Excellent communication and computer skills with EMR experience

Responsibilities

• Provide prescribed medical treatment and personal care to patients
• Administer medications as ordered and document administration
• Perform nursing assessments and routine health services per VA guidelines
• Operate diagnostic equipment (EKG, glucometers, etc.) and document results
• Maintain patient records in EMR and coordinate with VA and community services
• Deliver patient and family health education and support self-management
