Valor Healthcare, Inc. is seeking a passionate Licensed Practical Nurse to join our Granbury, TX clinic. You will deliver prescribed medical care and assist the core PACT team to develop individualized patient plans, while maintaining EMR documentation and patient confidentiality.

Qualifications

• Requires a diploma or certificate from an accredited practical nursing program.
• Minimum 1 year of LPN/LVN experience in primary or ambulatory care; government healthcare setting preferred.
• Must be licensed/credentialed in the state and in good standing.
• Current BLS certification; ACLS as required by contract.

Responsibilities

• Provide prescribed medical treatment and personal care per physician directions.
• Collaborate with PACT team and extended care teams to develop patient care plans.
• Document in EMR and maintain patient confidentiality.
• Administer medications and monitor patient responses per orders.
• Assist with patient education and self-management for wellness.
