Job summary

Virtua Health in Moorestown is seeking a Licensed Practical Nurse to support clinical triage, patient follow-up, and provider messaging. The LPN will assist in managing patient flow, prepare patients for exams, and perform phlebotomy, vaccines, and routine procedures under physician supervision.

Responsibilities include care coordination, education of patients, and post-discharge follow-up, with emphasis on safety, documentation, and collaborative team care in a busy outpatient setting.

Qualifications

• Experience in primary care preferred, minimum 6 months experience in a physician practice.
• Knowledge of minor surgical procedures and sterile technique.

Responsibilities

• Collaborates with Practice Leadership to conduct Daily Huddles and Weekly Care Team meetings, to review population health data.
• Prepares patient for exam by obtaining and documenting vital signs and chief complaint for provider exam.
• Provides general medical treatment such as administering medications and vaccines; performs phlebotomy as ordered.
• Performs diagnostic tests such as EKG and PFT, and documents results in the EMR.
• Coordinates test and referral tracking to support care coordination efforts.
• Leads patient education efforts and trains Medical Assistants in health coaching.
• Ensures outreach after hospital discharge or ED visits and schedules follow-up visits.
