Job summary

Creekside Center for Rehabilitation and Healing is seeking a Licensed Practical Nurse (LPN) to deliver post-acute care across physical, occupational, and speech therapy programs. Shifts include day, night, full-time, part-time and per diem with weekly or daily pay options and a highly competitive hourly rate up to $33.

You will administer medications, assist with admissions and discharges, document care in resident records, educate residents and families, and work with an interdisciplinary team

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
• Ability to administer medications and provide treatments.
• Adherence to infection control and facility policies.

Responsibilities

• Administer medications and treatments.
• Assist with admissions, transfers and discharges.
• Document resident care in medical records and department records.
• Educate residents and family members about treatment and status changes.
• Maintain confidentiality and privacy of residents.
• Collaborate with the interdisciplinary team to ensure quality care.
• Adhere to DOH standards and facility policies.
