Job summary

Creekside Center for Rehabilitation and Healing is seeking a dedicated LPN to deliver quality post-acute nursing care. Responsibilities include medication administration, patient admissions, transfers, discharges, and documenting care in medical records while educating residents and families.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents.
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc. as well as family members.
• Adhere to infection control policies and procedures and all facility policies.
• Consult with physician regarding plan of care and notify changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care by following DOH and facility standards.
• Assist with solving residents' problems by coordinating with the interdisciplinary team.

Education

Nursing degree or graduation from an approved LPN/RN program
