Job summary

Creekside Center for Rehabilitation and Healing is seeking a Licensed Practical Nurse (LPN) for immediate hire. Salary is up to $33 per hour with opportunities for day and night shifts, full-time, part-time, and per diem arrangements. Nursing degree or approved LPN/RN program completion is required. Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document care in resident medical records.
• Educate residents and families about treatments and status changes.
• Adhere to infection control and facility policies.
• Consult with physicians about plan of care and changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care per DOH and facility standards.
• Collaborate with interdisciplinary team to solve resident problems.
