SUMMARY OF JOB

The LPN Personal Care Nurse assesses the physical needs of residents and takes responsibility for their total care. This includes assessing changes in residents and responding appropriately based on those assessments. The nurse collaborates with other healthcare professionals, residents, and their families. Acts as a role model, recognizing the value of each person as an individual. Supports and promotes residents' strengths, abilities, capacity for social contribution, unique values, preferences, living habits, and autonomy of choice. Embraces a vision of aging that is meaningful and relationship-based.
