Londonderry Village in Palmyra, PA is hiring a Personal Care Charge Nurse for a full-time, evening shift. You will oversee the day-to-day Personal Care unit, provide hands-on nursing care, administer meds, and serve as a clinical resource for residents and staff.

You will lead and support the Personal Care team, supervise aides and technicians, and ensure care aligns with individual plans, LV policies, and safety procedures.

Qualifications

  • LPN license in good standing (PA).
  • First Aid and CPR certifications or willingness to obtain upon hire.

Responsibilities

  • Lead the Shift & Coordinate Care, ensuring safe, consistent resident care.
  • Maintain accurate narcotic counts and medication keys; complete required documentation.
  • Supervise Personal Care Aides and Medication Technicians, promote teamwork and safety.

If you’re a confident nurse who thrives in a fast-paced environment, enjoys leading others, and is passionate about creating a positive experience for residents, this is an opportunity to make a lasting impact.Your responsibilities will include:Lead the Shift & Coordinate CareReceive and provide thorough shift-to-shift reports, ensuring Personal Care Aides and Medication Technicians have the information needed to deliver safe, consistent care.Maintain accurate narcotic counts and medication keys while completing all required documentation.Supervise Personal Care Aides and Medication Technicians, ensuring resident care is delivered according to individual support plans, established standards, and LV policies and safety procedures.Deliver Exceptional Clinical CareConduct comprehensive resident assessments and daily rounds, monitoring for changes in condition and initiating appropriate interventions and follow-up.Prepare, administer, and document medications and treatments in accordance with physician orders and established nursing standards.Evaluate and document residents’ responses to medications and treatments and address concerns promptly.Recognize and respond to resident emergencies, including initiating appropriate life-saving measures consistent with the resident’s advance directives.Coordinate Communication & Follow-UpCommunicate significant changes in resident condition—including weight changes, medication errors, and abnormal test results—to physicians, responsible parties, and the Personal Care Administrator, ensuring timely follow-up.Accept and accurately transcribe physician orders and coordinate medication orders, laboratory testing, and retrieval of results as needed.Listen to concerns raised by residents, responsible parties, physicians, and staff; take appropriate corrective action and communicate concerns and follow-up to the Personal Care Administrator.Support Resident Transitions & Care PlanningAssist with developing, reviewing, and updating individualized resident support plans based on comprehensive assessments and changing needs.Coordinate admission, transfer, and discharge processes, including required documentation and communication with physicians, ancillary providers, residents, families, and other departments.Ensure resident information and care needs are communicated effectively throughout every transition.What You Bring to the TableLicensure: Current, unrestricted Pennsylvania Practical Nursing (LPN) license in good standing.Certifications: Current First Aid and CPR certification, or willingness to obtain upon hire.Regulatory Requirement: Must be at least 21 years of age in accordance with applicable licensing regulations.
