Job summary

CenterWell Home Health in Baton Rouge, LA seeks a Patient Services Coordinator, LPN/LVN to act as a vital link between clinical staff and patients, families, and caregivers. Responsible for scheduling, organizing and managing all aspects of a patient’s healthcare maintenance and treatment, with decisions guided by procedures and standards.

The role requires a Licensed Professional Nurse or LVN licensed in the state, plus 1 year of LPN/LVN experience. Preferred: 1 year in Home Health/Hospice.

Qualifications

• Licensed Professional Nurse or LVN licensed in the state of practice.
• At least 1 year of LPN/LVN experience.
• Preferred: 1 year of Home Health/Hospice experience.

Responsibilities

• Manage schedules for all patients, including supervisory visits.
• Schedule discharge visits and OASIS collection visits.
• Reschedule missed visits and reassign visits.
• Prepare and edit weekly agent schedules and review productivity.
• Assist with weekly case conferences.
• Receive calls from field staff and refer clinical questions to the Branch Director as necessary.
• May perform patient visits and participate in on-call rotation.
• Maintain the client hospitalization log and coordination notes.
• Send electronic logs to staff and distribute lab reports as appropriate.
• Verify visit paper notes in scheduling console as needed.
• Run scheduling reports (Agent Summary Report, Missed Visits).
• Assist with internal transfers of patients between branches.
• Additional duties may vary by branch census.
