Job summary

Qualifications

• Licensed RN or LVN in the applicable state.
• At least 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification is preferred.
• Valid state driver’s license and automobile liability insurance.
• Licensed in the state of employment if applicable.
• Excellent communication, strong organizational skills, and ability to manage multiple tasks.

Responsibilities

• Schedule management for all patients and reassign as needed.
• Coordinate clinical visits and ensure continuity of care.
• Initiate infection control reports and route to Case Manager.
• Maintain hospitalization log and distribute coordination notes.
• Assist with day-to-day office and staff management.
• Support field staff with calls and weekly case conferences.
