Qualifications

Responsibilities

• Perform initial, ongoing and functional health status assessment and document findings.
• Develop and coordinate care plans and promote cost-effective care.
• Evaluate responses to interventions and revise plans with the care team.
• Delegate tasks appropriately and ensure continuity of care.
• Participate in process improvement and act as patient advocate.
