Job summary

Intermountain Health in Grand Junction, CO is hiring a Licensed Practical Nurse (LPN) to support patient care under the supervision of RNs and physicians. You will assist with admissions, med administration, and daily living activities while documenting care plans and participating in education for patients and families.

The role requires an active LPN license, BLS-HCP, and possibly ACLS depending on unit. A strong focus on compliance, quality improvement, and teamwork is essential for this

Qualifications

• Current Licensed Practical Nurse (LPN) license in state of practice.
• Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire OR obtained within 60 days of hire with prior approval.
• Advanced Care Life Support (ACLS) is required if applicable to unit.
• IV certification may be required by facility.
• Colorado sites: Current IV Certification or obtain IV Certification within three months of hire.
• Montana sites: Completion of in-house education and competency sign-off within three months of hire.
• LPNs hired into hospital departments without long-term care/behavioral health must enroll and complete RN within 4 years.

Responsibilities

• Assist with admission and check-in, orienting patients and completing forms.
• Prepare and administer medications within scope of practice and assist with activities of daily living.
• Obtain and track specimens for lab analysis and document care and updates to care plans.
• Communicate with RNs, physicians, or practitioners about patient care and changes in condition; educate patients on health status and management.
• Follow legal/ethical standards, ensure regulatory compliance, document accurately, and participate in quality improvement initiatives; mentor staff as needed.
