Caretenders Home Health, part of the Optum family, seeks a Patient Care Manager Assistant in Columbus, OH. You will support the Patient Care Manager, coordinating a staff team to maintain high-quality in-home care for patients in your area.

Qualifications

• Current unrestricted LPN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification within 90 days of hire (preferred).
• Home care experience (preferred).

Responsibilities

• Coordinates a team of staff to ensure continuity of high-quality care.
• Prepares patient lists and documents for weekly case conferences.
• Provides direct patient care as needed per scope of practice and physician orders.
• Assists Patient Care Manager in documentation review and audits.
