Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing for a 3-year-old male child in Portage, PA. This part-time role on day shift offers 3 days per week, 8:00 AM–5:00 PM, with one-on-one pediatric care in a home setting.

The ideal candidate will have an active PA LPN or RN license, CPR certification, and good mobility, with at least 6 months of nursing experience. Join a supportive team dedicated to delivering quality pediatric care.

Qualifications

• Current LPN (or RN) license in PA.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform g-tube care/feedings.
• Monitor vitals.
• Trach/vent care/management.
• Perform respiratory assessments.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
