Qualifications

  • Active, unencumbered SC LPN/LVN license required.
  • Current CPR certification.
  • Maintain required continuing education/licensing and be in good standing with SC Board of Nursing.
  • Read, write, speak and understand English.

Responsibilities

  • Provide direct nursing care to residents.
  • Supervise day-to-day nursing activities by nursing assistants.
  • Work one required shift per week on 12-hour day or night shifts.
  • Direct nursing assistants and ensure compliance with facility policies and regulations.
  • Plan shift services, staffing, and assignments.
  • Provide shift reports and maintain documentation (census, medical, charting, incidents).
  • Admit, transfer, and discharge residents.
  • Administer physician-ordered medications and maintain narcotic records.
  • Coordinate nursing services with physicians and other departments.
  • Conduct rounds, monitor conditions, and notify physicians/family of changes.
  • Administer treatments (catheterization, tube feeding, wound care, ROM, etc.).
  • Review care plans and assist with MDS/RAPs/Triggers.
  • Participate in QA, infection control, safety, and disaster preparedness.
  • Train, orient, supervise, and evaluate nursing personnel.
  • Maintain safe, clean work areas and stock medications/equipment/PPE.
  • Participate in discharge planning and post-mortem procedures.

Job description

  • Provide direct nursing care to residents
  • Supervise day-to-day nursing activities performed by nursing assistants
  • Work one required shift per week on 12-hour day or night shifts
  • Direct nursing assistants and ensure compliance with facility policies, procedures, job descriptions, and regulations
  • Plan shift services, programs, activities, staffing, and work assignments
  • Provide written and oral shift reports and maintain census, medical, charting, and incident documentation
  • Admit, transfer, and discharge residents
  • Prepare and administer physician-ordered medications and maintain narcotic records
  • Review medication cards, transcribe physician orders, and maintain resident charts
  • Coordinate nursing services with physicians, resident services, social and activity programs, and other departments
  • Conduct rounds, monitor resident conditions, and notify physicians and family members of changes or incidents
  • Administer treatments including catheterization, tube feedings, suction, wound dressings, colostomy and drainage care, specimens, vital signs, and range-of-motion exercises
  • Review and update care plans and assist with MDS, RAPs, and Triggers
  • Participate in quality assurance, facility surveys, infection control, safety, and disaster-preparedness programs
  • Train, orient, supervise, and evaluate nursing personnel
  • Maintain clean, safe, sanitary work areas and adequate medication, equipment, supply, and PPE stocks
  • Participate in resident discharge planning and post-mortem procedures
Requirements
  • Nursing degree from an accredited college or university, or graduation from an approved LPN/LVN program
  • Active, unencumbered South Carolina LPN/LVN license required
  • Current CPR certification
  • Must maintain required continuing education/licensing
  • Must remain in good standing with the State Board of Nursing
  • Must be able to read, write, speak, and understand English
  • Knowledge and experience with PCC preferred
  • Ability to provide age-appropriate resident care
  • Knowledge of nursing and medical practices, laws, regulations, and nursing-care-facility guidelines
  • Leadership and supervisory ability
  • Ability to make independent decisions and work effectively with residents, personnel, families, visitors, and agencies
  • Must meet facility health requirements, including medical and physical examination
  • Ability to move, push, pull, and/or carry at least 25 pounds 50 feet
  • Ability to assist with resident evacuation during emergencies
