The Clare in Chicago is seeking a compassionate Part-Time LPN to join our Assisted Living/Memory Care Unit. You will deliver high-quality nursing care in a luxury residential setting and support residents' daily living activities.

This role requires an Illinois LPN license and CPR certification, with evening and night shifts on a rotating schedule. We offer competitive hourly pay and a benefits package.

Qualifications

• Current nursing license valid in Illinois.
• Valid CPR certification.

Responsibilities

• Assist residents with activities of daily living and monitor health changes.
• Develop and monitor care plans; re-assess residents as needed.
• Provide daily report to Health Services Director on conditions and supplies.
• Administer nursing care per physician orders and policies.
• Coordinate nursing care with other disciplines to ensure comprehensive care.

Shift: Week 1 - Friday 6:30 pm to 7:00 am, Week 2 - Monday 6:30 pm to 7:00 am

If so, then The Clare, the only Continuing Care Retirement Community in the Gold Coast area, wants you! We are seeking to fill a Part-Time LPN role in our Assisted Living/Memory Care Unit.

Here are a few of the daily responsibilities of an LPN:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
