The Clare, a luxury Continuing Care Retirement Community in Chicago's Gold Coast, is hiring a Part-Time LPN for our Assisted Living/Memory Care Unit. Join a compassionate team delivering outstanding care and customer service in a premier residential setting.

Responsibilities include assisting residents with ADLs, monitoring health changes, communicating with the Director of Health Services, and coordinating with other disciplines.

Qualifications

• Current nursing license valid in Illinois
• Valid CPR certification

Responsibilities

• Assist residents with Activities of Daily Living (ADL), family/social activities and promote independence.
• Identify resident medical and behavioral changes and develop and monitor care plans with re-assessment.
• Provide daily reports to the Director of Health Services about conditions, needs for nursing supplies, and staff performance.
• Deliver nursing care according to physician orders and established standards and policies.
• Ensure preventative, therapeutic and rehabilitative services and coordinate care with other disciplines.

Salary: $34.00-$35.00 per hour, plus night shift differential

Part-Time LPN

Shift: Week 1 - Friday 6:30 pm to 7:00 am, Week 2 - Monday 6:30 pm to 7:00 am

If so, then The Clare, the only Continuing Care Retirement Community in the Gold Coast area, wants you! We are seeking to fill a Part-Time LPN role in our Assisted Living/Memory Care Unit.

Here are a few of the daily responsibilities of an LPN:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician's orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
