Senior Lifestyle Corporation is seeking a Licensed Practical Nurse (LPN) for the Assisted Living community Lincolnwood Place in Lincolnwood, IL. This part-time, non-overnight (NOC) shift role delivers licensed nursing services, medication administration, and care coordination to residents with a focus on HEART values.

Requirements include 1+ year in senior care, active LPN license in the state, CPR certification, and English proficiency.

Qualifications

• Minimum 1+ years experience in senior care or related field.
• LPN license current in the practicing state; CPR certification required.
• English proficiency sufficient to perform duties and communicate in emergencies.
• Ability to work as part of a team and handle multiple tasks safely.

Responsibilities

• Administer medications in compliance with federal/state regulations and company guidelines.
• Manage admissions assessments and related documentation for new residents.
• Develop and update acute and long-term care/service plans.
• Track clinical data (e.g., wounds, infection control) under supervision.
• Provide health-related consultations to residents and staff as needed.
• Notify physicians and families about health changes with proper documentation.
• Respond promptly to emergencies and coordinate with other staff.
• Educate residents and staff on health topics and maintain confidentiality.

This position is responsible for providing licensed nursing services to residents, following the guidelines provided by Senior Lifestyle Corporation.

Pay Rate: 36.00 to 38.00 Hourly

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
