Job summary

The Clare in Chicago, IL is seeking a hospitality‑focused Licensed Practical Nurse to deliver direct nursing care to residents and supervise care provided by our Quality‑of‑Life Specialists/Certified Nursing Assistants. The position supports an engaging, community‑driven environment with a rotating weekend schedule.

This full‑time evening shift (6:30 pm to 7:00 am) emphasizes compassionate care and teamwork, with opportunities to contribute to a vibrant senior living community.

Qualifications

• Current nursing license valid in Illinois.
• Willingness to be available for any/all emergencies regarding the community.

Responsibilities

• Assist residents with ADLs, social activities, and promote independent living.
• Identify resident medical/behavioral changes; develop and monitor care plans and re-assess.
• Provide daily reports to the Director of Health Services about conditions, staff performance, resident needs, and nursing supplies.
• Provide nursing care per physician orders and established standards and policies.
• Coordinate nursing care with other disciplines for preventative, therapeutic and rehabilitative services.

The Clare is recruiting for a hospitality focused Licensed Practical Nurse to join our team! This position provides direct nursing care to residents and delegates and supervises the care provided by our Quality-of-Life Specialists/Certified Nursing Assistants.

Here are a few of the daily responsibilities of an LPN:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician's orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
