Job summary

Senior Community Care of North Carolina is seeking an LPN for the Med Room at our PACE clinic. Schedule: Monday-Friday 8:30 AM-5:00 PM with on-call rotation. Salary range $29.00-$35.50 per hour.

The role involves in-clinic and in-patient/home nursing care, medication administration, and collaboration with the interdisciplinary team. Requires Graduation from an accredited nursing program, active LPN license, CPR certification, and at least one year of elderly care experience.

Qualifications

• Graduate of an accredited nursing program.
• Current Practical Nurse license (LPN).
• CPR certification required.
• Minimum of one year experience with frail or elderly.
• Must have valid driver’s license, proof of insurance and transportation.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Completes basic assessments and assists PACE RNs as delegated.
• Participates in Interdisciplinary Team meetings and plan of care decisions.
• Performs nursing care in PACE clinic and homes.
• Administers prescribed medications and treatments per nursing standards.

Responsibilities

• Care for participants in the PACE clinic and in homes.
• Assist with assessments and care planning with the PACE team.
• Administer medications and follow nursing protocols.
