Job summary

Senior Community Care of North Carolina, part of Volunteers of America, seeks an LPN for the Med Room at our PACE clinic. Durham area. Schedule: Mon-Fri 8:30 AM5:00 PM with on-call rotation.

You will administer medications, support care plans, and coordinate with the interdisciplinary team in clinic and participant homes. We offer a comprehensive benefits package including a 403(b) plan, career scholarships, ongoing training, medical/dental/vision, and paid time off.

Qualifications

Graduate of an accredited nursing program.
Current Practical Nurse License (LPN).
CPR certification required.
At least 1 year experience with frail/elderly population.
Valid driver7s license, proof of insurance and transportation.
Medical clearance for communicable diseases and up-to-date immunizations.

Responsibilities

Performs nursing care in clinic and participant homes per LPN scope of practice.
Administers prescribed medications and treatments.
Communicates participant changes to primary care providers and interdisciplinary team.
Completes basic assessments and assists RNs as delegated.
Maintains regular attendance at Interdisciplinary Team meetings.
