Job summary

Horizon Health is seeking an LPN for our Foundations Health clinic in High Point, NC. The OTP LPN coordinates and evaluates patient care through collaboration with the interdisciplinary team.

Responsibilities include dispensing methadone and other meds per physician orders, recording patient information, conducting brief assessments, and participating in UDS surveillance and team meetings. This per diem role offers growth with Horizon’s national network.

Qualifications

• Current NC LPN license or eligible to practice in NC.
• Minimum 2 years experience in an addiction care setting (Inpatient, Detox, OTP) preferred.

• Accurately record all patient information in the patient record.
• Completes brief assessment of patient's mental and physical well-being, monitoring for signs/ symptoms associated with licit and illicit drug use.
• Participates in UDS surveillance, per policy.
• Coordinates medication reviews and staffing with agency physicians.
• Completes all required controlled substance inventories to ensure accurate medication counts.
• Conducts patient education programs according to needs identified, including health and wellness education.
• Participates in treatment team meetings, providing feedback to the interdisciplinary team.
