Job summary

Horizon Health in High Point, NC, is seeking an LPN for the Foundations Health clinic on a per diem basis. The LPN will delegate, coordinate and evaluate patient care through the nursing process and OTP standards with an interdisciplinary team.

Responsibilities include dispensing medications, documenting records, assessing wellbeing, participating in UDS surveillance, and coordinating reviews with physicians. Weekend shift on Saturdays from 5a to 9a. EOE.

Qualifications

• Current NC LPN license.
• Minimum 2 years experience in an addiction care setting (Inpatient, Detox, OTP) preferred.

Responsibilities

• Dispenses Methadone and other medication, per physician orders and clinic policy.
• Accurately record all patient information in the patient record.
• Completes brief assessment of patient;s mental and physical well-being, monitoring for signs/ symptoms associated with licit and illicit drug use.
• Participates in UDS surveillance, per policy.
• Coordinates medication reviews and staffing with agency physicians.
• Completes all required controlled substance inventories to ensure accurate medication counts.
• Conducts patient education programs according to needs identified, including health and wellness education.
• Participates in treatment team meetings, providing feedback to the interdisciplinary team.
