Qualifications

• Current State LPN License.

Responsibilities

• Assists with data collection and provides feedback to RN from multiple sources to inform treatments and nursing interventions.
• Assists with discharge planning and documents patient responses to interventions.
• Delivers care per plan of care with safety, accuracy, and cultural sensitivity.
• Educates patients and families about needs identified during admission and hospitalization.
