Job description

Job Summary: Will be a Licensed Practical Nurse performing duties as a patient navigator, guiding incoming calls and facilitating the entrance of new patients into the program. Provides patient education, promotes understanding of conditions, and ensures ongoing continuity of care. The Nurse Navigator, working in a team-oriented environment, is accountable for delivering care using skills learned in clinical and didactic settings. Assists other team members and provides clinical support to providers and patients to aid in diagnosis and treatment.

Job Duties:

  1. Utilizes clinical knowledge to facilitate patient introduction to the program.
  2. Evaluates patient needs primarily via telephone or documentation and documents findings.
  3. Collaborates with healthcare providers to coordinate and implement patient care plans, including evaluation, treatment, and surveillance.
  4. Communicates effectively with patients through mail, phone, and in person regarding their care.
  5. Ensures relevant patient information is available for provider interactions.
  6. Coordinates patient needs and activities within the program.
  7. Serves as the point of contact for providers.
  8. Coordinates patient activities such as support groups and educational sessions.
  9. Provides limited patient and family education based on individual needs, either in person or by phone.
  10. Maintains systems to ensure timely follow-up and surveillance.
  11. Acts as a liaison with Scheduling Services for appropriate patient scheduling.
  12. Develops and implements patient screening tools and clinic procedures.
  13. Assists in developing and implementing navigational procedures.
  14. Investigates patient concerns related to appointments, communication, and billing.
  15. Maintains clinical supply inventory.
  16. Conducts medical record reviews and reports findings to physicians.
  17. Provides nursing care and assists physicians and procedures.
  18. Inspects and maintains equipment to ensure proper operation.
  19. Performs ongoing maintenance of exam and treatment rooms.
