Job summary

Qualifications

• LPN with at least 2 years of experience; cardiovascular and adult echocardiography experience preferred.
• Strong communication and teamwork skills are essential.
• Ability to organize care plans and document effectively.

Responsibilities

• Utilize clinical knowledge to facilitate patient program entry and assess needs primarily via phone or documentation review.
• Coordinate care plans, communicate with patients, and act as main contact for providers.
• Assist in scheduling, develop patient screening tools, and improve navigational procedures.
• Address patient concerns, maintain clinical supplies, and review medical records for physician reporting.
• Provide nursing care, monitor equipment, and maintain exam and treatment areas.
