Prominence Health is seeking an LVN Navigator to serve as a liaison among the patient, primary care provider, and the multidisciplinary team to develop an optimal plan of care. The LVN Navigator conducts clinical and social assessments across settings including phone, ED, primary care, SNFs, and sometimes home visits.

Formación

• Licensed LPN/LVN in Texas and graduate from an accredited program.
• Ability to document in EHR and use Microsoft Office.
• Effective communication in English, both written and spoken.
• Experience in care coordination or transitions of care is a plus.

Responsabilidades

• Liaise between patient, PCP, and multidisciplinary team to plan care.
• Assess clinical and social needs across multiple care settings.
• Build rapport with providers, patients and families to expedite transitions.
• Communicate treatment plans and status to caregivers during transitions.
