LiveWell Homecare Agency seeks a Licensed Practical Nurse to act as a Patient Navigator, guiding new patients and ensuring continuity of care. You will assess needs, educate families, and coordinate with providers to smooth entry into services.

In this clinical-focused role, you’ll review records, assist physicians, manage program activities, and develop screening tools to address access and billing concerns, while upholding organizational policies.

Qualifications

• Minimum of 2 years LPN experience.
• Graduate from Specialty Training Program in Nursing.
• Clinical knowledge to facilitate patient entry and ongoing support.

Responsibilities

• Assess patient needs via phone or documentation, coordinate care with healthcare providers.
• Communicate effectively with patients, manage program activities, and serve as liaison for scheduling and support services.
• Perform medical record reviews, assist physicians, maintain clinical supplies, and ensure proper functioning of equipment.
• Develop screening tools and procedures to enhance patient navigation and address patient concerns related to access and billing.
