Redstone seeks an LPN Clinical Liaison (Health Navigator) for full-time work in Westmoreland County. You will serve as the primary contact for clients in Greensburg and nearby communities, coordinating services and ensuring satisfaction.

The role includes collaboration with physicians, hospitals, and referral sources. Responsibilities cover client education, marketing activities, and participation in on-call programs.

• A valid LPN or RN license in Pennsylvania.
• Previous experience in home health care, hospice, or community health.
• Experience in sales and marketing preferred.

Shift: Monday through Friday, 8:00 am - 4:30 pm, Can be flexible!

What will I do as an employee with Redstone?

As the LPN Clinical Liaison (Health Navigator), supporting the Redstone mission will include the following responsibilities:

• The position reports directly to the Director of Health Navigation. They serve as the main point of contact for clients within the 3 Redstone Highlands Communities in Murrysville, North Huntingdon and Greensburg to understand their needs, providing support, and ensuring client satisfaction. The position may be responsible for conducts clinical rounds with physicians and visits within the local hospital systems. They promote the image and philosophy of Redstone@Home.
• Customer Service:
  • Conduct regular meetings with referral sources.
  • Identify needs of community seniors and referral sources.
  • Coordinate education and program services.
  • Participate in networking activities and events.
• Quality and Compliance Management:
  • Collaborate with internal teams to address client concerns.
  • Support quality and regulatory compliance with partner facilities.
  • Maintain knowledge of funding and government regulations.
  • Utilize technology to plan and document marketing activities.
  • Participate in department and program planning meetings.
• Other Responsibilities:
  • Provide clinical staffing relief for Home Health.
  • Participate in marketing/sales activities.
  • Participate in on-call programs.
  • Perform other duties as assigned.

What do I need to qualify for this role with Redstone?

• A valid LPN or RN license in the state of Pennsylvania.
• Previous experience in home health care/hospice/community health.
• Experience in sales and marketing preferred.
