Signature HealthCARE is hiring LPNs in Arkansas to provide skilled nursing care at Hillcrest and Hermitage Care & Rehab. You’ll administer medications, support ADLs, and supervise nursing staff to maintain high standards of patient care.

Requirements include an active LPN/LVN license and at least one year of clinical nursing experience, with LTC experience preferred. Excellent documentation, EHR familiarity, and teamwork are essential for success.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Supervisory/charge nurse experience preferred; strong documentation skills.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living while ensuring dignity and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents' condition and communicate changes to physicians and families.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and staff education.
