Signature HealthCARE in Kentucky is hiring Full-Time LPNs at Hillcrest & Hermitage Care & Rehab. We offer a $5,000 sign-on bonus paid over 26 pay periods and a night shift differential of $8/hour to eligible evening/night staff.

You'll provide direct nursing care, administer medications, supervise nursing assistants, and participate in care planning, with opportunities for professional growth in a supportive team, modern EHRs, and a commitment to compassionate, person-directed care.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) with a focus on comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shift per policy.
• Monitor residents and report changes to physician or guardian.
• Develop and review individualized care plans with the interdisciplinary team.
