Job summary

Signature HealthCARE seeks a compassionate LPN in Indiana to provide skilled nursing care and support to residents at Hillcrest and Hermitage Care & Rehab. Join a team committed to person-directed care and strong interdisciplinary collaboration.

You will handle medications, ADLs, and care plan implementation while supervising nursing staff. A licensed LPN/LVN in Indiana with CPR/BLS preferred is desired, with a night shift differential and a $5,000 sign-on bonus offered.

Qualifications

• LPN/LVN license active in the state of practice (Indiana).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Experience in a supervisory or charge nurse role preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor and observe residents’ status; notify physician of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and staff education.
